Fresh embryo transfer remains 40-60% of cycles in experienced centres, and a large share of failures sit in the early luteal phase. The talk walks through LH and progesterone dynamics after HCG and agonist triggers, the progesterone 'tsunami' on days 2-6, and the moment the corpus luteum declines and additional luteal-phase support starts to matter.
A mid-luteal serum progesterone below approximately 50 ng/mL on the day of blastocyst transfer is associated with a 12-16% drop in live birth in two single-centre studies and one large retrospective Chinese cohort. The GnRH-agonist trigger gives an LH peak about 75% lower than the natural surge and lasts only 24-28 hours, so unsupplemented agonist fresh cycles historically lost up to 80% of pregnancies. The 6,500 IU HCG trigger covers the early luteal phase for about 6.5 days; rescue with additional progesterone is what protects implantation around days 4-6.
Routine vaginal progesterone supplementation in fresh cycles remains the baseline. A subgroup with low corpus-luteum response — often despite many follicles — benefits from rescue with rectal or subcutaneous progesterone. Mid-luteal serum progesterone monitoring is a reasonable add-on where corpus-luteum function is doubtful; randomised rescue trials in fresh cycles are in progress.
Cut-off data are still single-centre and need replication. Routine progesterone measurement in true natural cycles is not advised because of 30-minute pulsing. Oral dydrogesterone is harder to titrate against a measurable serum level.
Why does luteal progesterone matter just as much in fresh cycles as in frozen? A walk-through of trigger choice, mid-luteal cut-offs and rescue strategies for the suboptimal corpus luteum — built on more than 15 years of randomised trials.
